Using a post-stroke checklist and a functional assessment and referral for relevant interventions at the follow-up at 3-6 months is provided to at least 90% of patients.

Current status

Only 9 countries provide a structured follow-up based on a post-stroke check-list in at least 90 % of patients and this data is almost exclusively based on estimates. The needs at follow-up include – but is not necessarily limited to – secondary prevention, new complications including spasticity or pain, and need for further rehabilitation. To ensure relevant action to these quite comprehensive needs in a condition as frequent as stroke requires a structured approach – a check-list.

SAP-E recommends that a post-stroke check list is included in the National stroke plan, also to ensure that referral for relevant interventions are facilitated.

The target is structured and comprehensive follow-up based on a post-stroke check-list.
