WORKSHOPS

Participants will be divided into groups to discuss the following topics:

  • Topic 1- Continuity in the chain of care, from acute care to rehabilitation and life after stroke
  • Topic 2- Ensuring optimal secondary prevention before discharge
  • Topic 3- Addressing social determinants of health, to improve access, implementation of SAP-E and good outcomes
  • Topic 4- Co-operation of professionals and Stroke Support Organisations
  • Topic 5- Challenges in using Real-time monitoring of Rehabilitation, PREMs and PROMs. Foundation to truly achieve patient centered care
  • Topic 6- The importance of national coverage in quality registration to achieve equitable service

For each topic, Coordinators are invited to prepare the following questions:

  1. What are the major gaps in my national stroke system today?
  2. What priority would have the greatest impact if implemented within 2-3 years?
  3. What might be the main barriers (political, organisational, financial) to achieve this and how could they be addressed?
